Service Description: Supports for people with Alzheimer's disease and other dementias, and their caregivers * helps people remain independent in their own homes for as long as possible * referral to other community services

Elgin Satellite Office - Supports for people with Alzheimer's disease and other dementias, and their caregivers * 3 M Clinic
 
Medic Alert Safely Home - Nationwide program designed to help identify the person who is lost and assist in a safe return home using engraved identification such as bracelets, necklaces, watches and more * allows police and emergency responders to identify the person who has wandered
 
Resource Library - Resources on Alzheimer's disease and related dementias available such as books, magazines, videos, Alzheimer's disease information, personal stories, and reference material
 
Support Groups - Peer support groups, education, and training for family members, service providers, and caregivers * sessions may include information such as advanced directives and senior safety
  • Brain Wave - Support/educational group for people with dementia in the early stage of the disease
  • Wake Up Wednesday - A group for people with mid-stage dementia * 3 hour group with planned activities * weekly meetings
  • Women's Caring for Yourself

  • mixed caregiver groups
  • evening group for younger caregivers
