Service Description: Supports for older adults, and adults with disabilities and their families * helps people remain independent in their own homes for as long as possible * referral to other community services * registered charitable status
Exercise and Falls Prevention Program - Twelve week series of classes for adults aged 60 years and older * classes combine exercise and education and are designed to help older adult stay active, independent and mobile
Home Maintenance and Repair Brokered Service - Registry of screened workers who will help clients with yard maintenance such as grass cutting and gardening * window cleaning * snow  removal and more
Phillips Lifeline Program - Medical alert systems * allows older adults and people with disabilities to call for assistance using a help button * 24-hour emergency response * the new GoSafe System offers personal security while users travel anywhere in Canada
Stroke Specific (Recovery) Exercise Program - Brockville - Exercise classes for stroke survivors * caregivers welcome * based on the guidelines for community based exercise programs for people with stroke
Stroke Specific (Recovery) Exercise Program - Perth - Exercises program for stroke survivors with mild to moderate impairment * based on the Ontario Stroke and Network Recommended Guidelines
Van Just For Seniors - Senior's outings * 1-4 hour trips * van picks seniors up at their door within Westport, Portland, Elgin and Lombardy every Mon, Wed, Fri to go to Smiths Falls for

Volunteers are needed for many CPHC programs and work in different areas throughout Lanark, Leeds and Grenville * contact
