The Fundamentals ELIGIBILITY CRITERIA
 
The core Fundamentals, an eight week program, is intended for those working in the health system who have an interest in developing his/her capacity related to hospice palliative care. Roles can include:
 
·  Personal Support Workers
·  Recreational therapists
·  Housekeeping
·  Dietitians
·  Social Workers
·  Developmental Service Managers
·  Students (e.g. health science, PSW, RN, Gerontology)
·  Case coordinators
·  Directors of Care
·  Health Administration
·  Occupational Therapists
·  Speech Language Professionals
·  Physical Therapists
·  Nurse Practitioners
·  Registered Nurses
·  Registered Practical Nurses
·  Pharmacists
·  Physician / Family Health Team staff
·  Specialty care services (e.g. renal, obstetrics, ICU, Acute Care)
·  Mental health case managers
·  Emergency Medical Services
·  Volunteers




Following the completion of this course, the learner will:
  • Demonstrate sensitivity, understanding and respect for the individuality of the person, family and team/caregivers.
  • Identify and understand issues associated with illness and bereavement.
  • Effectively communicate with the person, family and team.
  • Recognize and report changes in any of the Domains of Issues associated with illness and bereavement.
  • Provide hospice palliative support care strategies.
  • Actively learn and develop personally and contribute as part of a team and organization.
  • Demonstrate sensitivity to the impact of personal attitudes, behaviours and values on the illness experience of the person, family and team/caregivers.
  • Learn about communication, assessment tools, ‘yellow folder’, Symptom Response Kit (SRK), End of Life symptoms, grief, Medical Assistance in Dying (MAID) & care for the caregiver.
