South East Local Health Integration Network

The South East Local Health Integration Network (LHIN) is one of 14 networks established by the province to plan, manage, and fund the health care system at the local and regional level. Promoting integration and networking among our health care resources is already showing results. The South East region extends from Brighton on the west to Prescott and Cardinal on the east, north to Perth and Smith Falls, and back to Bancroft.

Position:  Planner

Region:  Hastings


This position is responsible for development, implementation and /or management of strategic implementation initiatives to promote efficiencies and high quality system and patient /client outcomes. Supports or leads better coordination of health care services in the region fostering a culture of collaboration and integration to achieve provincial targets.

More specifically, in this role you will:

  • Provides leadership for development of innovative approaches for provider partnerships and selects among alternative approaches to achieve LHIN targets.
  • Provides leadership for the development of a culture within the region that reflects the LHIN’s role in driving quality improvement, integration and implementation locally.
  • Engaging regionally and provincially in conjunction with other LHIN staff to further LHIN and MOHLTC priorities.
  • Contributes to the delivery of LHIN accountabilities with respect to health care services that report to the LHIN (i.e., engagement with providers and related stakeholders, development of accountability indicators, funding request analysis, and program/project evaluation).
  • Providing leadership, direction, and support to other areas for which the planner is responsible including specialized programs, support services and unique patient population initiatives, fostering a culture of commitment and teamwork.
  • Actively contributes to best practices in patient-centred delivery of health care in the LHIN. This is achieved through the provision of expertise in the identification, analysis, evaluation, action- management and monitoring of sector issues.
  • Contributes to effective management of risk at the LHIN by applying leadership in the identification, assessment and prioritization of perceived and/or actual sector risk situations.
  • Actively participates in LHIN community engagement opportunities as they relate to the respective area of responsibility. In this context, will provide their leadership in the use of processes to inform/educate, gather input, consult with, and involve stakeholders.
  • Performs report writing as appropriate including preparation of presentation materials, briefing notes, etc.
  • Manages project plans for integration and implementation initiatives.
  • We seek a profile that includes:
  • Master’s Degree in health administration, health planning, business administration or relevant field
  • Demonstrated leadership experience within health care. Involvement with initiatives aimed at improving integration and implementation within the health care system across multiple sectors.
  • Experience in project administration/management and project management methodology.
  • Demonstrated experience and success in project management involving diverse groups.
  • Demonstrated experience working effectively in partnership with broad range of individuals and organizations of diverse backgrounds, preferably within the health care sector.
  • Superior interpersonal, facilitation and relationship management skills.
  • Excellent communication skills to effectively communicate with all staff at all levels, including the general public.
  • Experience in conflict resolution and management.
  • Strong teamwork and customer service skills.
  • Minimum of three years’ experience
  • In-depth understanding of the Ontario health care system, its stakeholders, programs and issues.
  • In-depth understanding of the community health care system
  • Proficiency with PC-based hardware/software and inter / intranets
