Loyalist Family Health Team

The LFHT provides high quality enhanced primary health care while maintaining strong doctor-patient relationships. The LFHT consists of physicians, nurse practitioners, nurses, allied health professionals and administrative support working collaboratively, each utilizing their experience and skills to provide enhanced care to the Team’s 11,000 patients. The Registered Nurse (RN) will work full-time to provide high level primary care to the patients of the LFHT while also being responsible for program planning, implementation and assessment.

Description:  Main Responsibilities:

  • ssessment, screening, education and support to assist patients to achieve and maintain their optimal level of health 
  • esponsible for the development, implementation, management and assessment of the diabetes and smoking cessation programs 
  • rovides direct patient care within the RN scope of practice while respecting LFHT Medical Directives 

Education, Experience and Skills:
  • Baccalaureate Degree in Nursing 
  • Current unrestricted registration with the College of Nurses of Ontario 
  • Professional malpractice insurance 
  • At least three (3) years clinical experience (primary care experience an asset) 
  • Experience working with both individuals and in groups 
  • Skilled in effective communication, both written and verbal with an ability to provide clear and concise information while using tact and consideration Able to actively listen, ask for opinions and accept feedback with a focus on self and organizational improvement 
  • Must maintain confidentiality at all times related to patient, staff, and organizational information 
  • Comfortable working with patients in a non-judgmental and welcoming fashion 
  • Work effectively both independently and as part of a team 
  • Certified Diabetes Educator (considered an asset) 
  • Memory clinic training (considered an asset) 
  • Smoking cessation training and experience (considered an asset) 

Job Requirements:
  • Must be able to travel 
  • Must be able to occasionally work outside of regular business hours
