Older adults and adults with disabilities * unable to access public transportation because of short or long term physical limitations * require escorts during the trip because of the frailty or cognitive impairment * specialized transit is not available or the individual does not meet the eligibility criteria * public transportation is not available and friends and relatives are not available or able to provide transportation
