Service Description: Supports and assistance to eligible youth leaving care to identify, access and navigate adult service systems relevant to their needs when transitioning to adulthood * emotional support

Eligibility / Target Population: Voluntary Youth Service Agreement (VYSA) participants, Crown Wards, and youth who receive support through the Continued Care and Support for Youth (CCSY) Program * Crown Wards do not have to be receiving services from Children’s Aid Societies to access support from the Youth In Transition Program
