Stroke prevention and case management for high-risk stroke patients * access to diagnostic testing, medical and surgical management and counseling in lifestyle change * facilitates planning and implementation of primary and secondary stroke prevention strategies
Works with the Regional Stroke Centre and other partners to provide leadership in coordinating stroke care
