Service Description:	Stroke care for patients in the acute phase * the team is made up of physicians, nurses, physiotherapists (PT), social work (SW), occupational therapist (OT), speech pathoogists (SLP), registed dieticans (RD), as well as other support services

Eligibility / Target Population:	Patients requiring admission following a Transient Ischemic Attack (TIA), Ischemic, or hemhoragic stroke
