Service Description: Delivers centrally coordinated, community based programs including:   
  • Community Service Supports - supports for elders including medical transportation, friendly visits, referrals, help with meals, homemaking and caregiver support services 
  • Community Wellness Worker program - helps to reduce violence in the family and provides peer support referrals and advocacy
  • Community Action Programming for Children - group activities for children 0-6 years of age
  • Metis Healthy Baby Healthy Children - parenting and child development programs
 Programs focus on: 
  • healthy parenting
  • early childhood development
  • chronic disease management
  • independent living
  • prevention of addictions
  • problem gambling
  • mental wellness
  • group children activities
Ontario Telemedicine Network (OTN) to provide psychiatry and other health care specialists with a remote connection to patients and clients
