Service Description:

Delivers centrally coordinated, community based programs including:
- Community Service Supports - supports for elders including medical transportation, friendly visits, referrals, help with meals, homemaking and caregiver support services
- Community Wellness Worker program - helps to reduce violence in the family and provides peer support referrals and advocacy
- Community Action Programming for Children - group activities for children 0-6 years of age
- Metis Healthy Baby Healthy Children - parenting and child development programs

- chronic disease management
- independent living
- prevention of addictions
- problem gambling
- mental wellness
- group children activities
Ontario Telemedicine Network (OTN) to provide psychiatry and other health care specialists with a remote connection to patients and clients
