Service Description: Support and stability during the transition process from one mental health agency to another * assists with emotional well-being and day-to-day activities

Application: Call * referral not required
Lanark, Leeds and Grenville services are available through the Lanark Leeds and Grenville Addictions and Mental Health agency
Eligibility / Target Population: Coordinates supports for youth between 16-24 years of age who are struggling with mental health and/or addiction issues
