Service Description: Independent witness program connects patients wanting to request  medical assistance in dying with trained independent witnesses who will sign a Clinician Aid A form affirming the individual's intention * volunteers may also serve as a proxy for the patient if the patient is too frail or physically unable to sign the form on their own * volunteers will visit the patient at home or meet on-line.
Note: Federal legislation changes in March 2021 resulted in the need for only one independent witness and that witness may be a health professional or community care worker or staff person at a hospital or long term care facility. 

Fees: Free
Application: Call, no referral required from a health care provider
Eligibility / Target Population: Determined by a patient's doctor or nurse practitioner
