Service Description: Regional outreach for specialty mental health supports for adults * intake assessment * supportive counselling * occupational therapy and social work support * with a goal  of transitioning the stable patient's care back to the family physician

Fees: Most services are covered by OHIP
Application: Stable moderate to severe mental illness of at least two years of duration and whose last psychiatric admission was greater than six months prior to referral and has an established contact with a family physician
Eligibility / Target Population: Adults 18 years and older
