Service Description:

Individualized programs using a participate-to-learn approach * the model rests on roles as goals * learning by experience in real-life contexts and the use of personal and environmental supports to enable participation * newly referred clients may participate in a group for newcomers to learn about acquired brain injury and identify individual service goals

Fees:
|
Subsidized by the Ministry of Health * fees are charged for participants who have access to private rehabilitation funding

Referrals can come from service providers, individuals, and family members * a written Referral Form with medical documentation of an acquired brain injury is required Referral form link Community Brain Injury Services contacts the person within one week of referral
