Primary health care interdisciplinary team focused on family and community health * coordinates patient care in order to assess and treat illness * team includes physicians, nurse practitioners, registered nurses, registered practical nurses, certified respiratory educator, certified diabetes educator, pharmacist, social worker and registered dietitian
