Eligibility / Target Population:
|
People aged 16 or older * understands the nature of their disability * requires lifelong assistance * able to direct their own care * able to have medical/professional needs addressed by existing community health providers * may be left unattended during periods when services are not scheduled * has a valid Ontario Health Care
