Service Description: The Cardiac Rehabilitation Program provides rehabilitation to patients recovering from a recent cardiac event or diagnosis. The program consists of diagnostic testing, assessment and monitoring; participation in personalized exercise groups and education programs with access to a multi-disciplinary health care team including a Cardiologist, Physiotherapist, Physiotherapy assistant, Nurse, Dietician and Social Worker.

The goal of the program is lasting lifestyle change and a return to physical activity, in order to help you reduce cardiovascular risk factors and give you the tools and knowledge to improve your health while recovering from a cardiac event or adjusting to a new cardiac diagnosis.

Application: Physician Referral
Eligibility / Target Population: Adult patients with diagnosed Cardiac disease or condition, who are stable enough to participate in vigorous exercise. Participants must be motivated to both participate in on-site programing, and modify lifestyle to decrease cardiovascular risk factors. Patients must be able to attend 2-3 times per week for 8-12 weeks.
