Service Description: Health centre focused on family and community health * health professionals assess and treat non-life-threatening injuries or illnesses * referrals to other local health services and personal development groups

Doctors, nurse practitioners, nurses and other healthcare professionals work together to provide primary health care services including physical assessments and treatment, nutrition support and social work counselling to registered clients

Community Programs and Services - Group programs and services to improve the health and well-being of Centre clients and people in the community * Centre staff also help people find community resources to meet their basic needs
 
Community Resource Assistance - Assistance for individuals and families needing support or advocacy with day-to-day challenges
 
Managing Chronic Conditions - Information and skills-based groups and peer-support groups to help people living with chronic pain, diabetes and other chronic conditions * scheduled throughout the year
 
Mental Health - Groups provide psychoeducation, social and self-help opportunities for individuals to work in collaboration with others addressing how to manage mental health symptoms, learn coping strategies and connecting with others whom identify having lived experience
 
Nutrition and Food - Community nutrition programs are scheduled throughout the year * they may include free meal programs, basic cooking skills, understanding nutrition, weight management, and programs for purchasing low cost fresh fruits and vegetables
 
Nutritional Counselling - Information and education from a clinical dietitian
 
Physical Activity - Community physical activity programs * walking groups and seated exercise
 
Social Programs - Community social programs * to help adults get out of the house, meet others, and reduce loneliness * arts and crafts
 
Telemedicine - Coordinating Ontario Telemedicine Services (OTN) throughout Hastings and Prince Edward Counties
 
Thrive - Support for mothers with addictions - Supports for pregnant or parenting women (children ages 0-6) struggling with substance use 
  • case management and a single coordinated care plan 
  • in-hospital and in-home support visits
  •  advocacy and support with social service agencies 
  • parenting support and education
