Fees:

Fees for infants ages six weeks to 18 months are $53, and toddler ages 18 months to two and a half years are $39, and preschool children ages two and a half to six years are $38 * Subsidies may be available

Parents fill out an application form including immunizations, allergies, and contact infomation.
