Medical health care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, nurses, nurse practitioner, dietitian, social worker and occupational therapist
- referral to specialty clinics

- dietary counselling
- specialty group programs
- sexual health services including family planning, pregnancy tests and sexually transmitted diseases
- well baby visit program in the clinic at 18 months
