Service Description: Rapid access to assessment, diagnostic testing and treatment for patients identified as having transient ischemic attacks ( TIAs ) / minor stroke or high risk factors * clinic offers:
  • consultation with an internist
  • a Registered Nurse assessment
  • consultation with a Registered Dietician
  • programs to assist optimal wellness
  • referral to other disciplines as necessary such as:
    • day rehabilitation
    • vascular surgeon
    • physiotherapy
    • occupational therapy

Fees: Most fees are covered by OHIP
Application: Physician referral * referrals may be faxed
Eligibility / Target Population: Patients who have had a transient ischemic attack * stroke or who have high risk factors
