Eligibility / Target Population: Individual diagnoses with 4+ chronic or complex conditions, but this is not intended to preclude practitioners from using their clinical judgement to identify and to include patients that have fewer than 4 of the conditions but for whom the complexity of their medical and social circumstances deems Health Links individuals the intensity of Health Links support
