Service Description: Access to health supports in the community

Care coordinators assess eligibility and create care plans and coordinate service provision to patients that need health supports in the community * service provision may include
  • nursing
  • personal support
  • physiotherapy and occupational therapy
  • speech and language therapy
  • social work
  • nutrition and food counseling (dietitians)
  • medical supplies and equipment
Long-Term Care Placement - Eligibility determination for short and long stay admission to long-term care facilities and prioritize and manage the admission process
Mental Health and Addictions School Program - Assessments, interventions and support to students and families with mental health and or addiction issues
Nursing Clinics - Nursing clinics located throughout the South East region provide nursing care to out-patients by addressing the specific health-care needs of LHINS patients such as IV therapy, wound care, or rehabilitation therapy
Palliative Care - Support and care for patients at home with a life-limiting illness * specialized hospice palliative care nurse practitioners
Palliative Care Education - Scheduled group classes at various locations in the South East region
Palliative Pain and Symptom Management Consultation - Support for front line care providers who are supporting people and families living with a life-limited illness
Respite - In-home or out of home temporary relief for caregivers
School Health Supports - Health services in school for students who require medical and/or rehabilitative assistance to enable them to attend and participate in school
Specialized Information and Referral - Staff are available by telephone to connect people with the health and support services they are looking for in the community
