Service Description: Rapid access to assessment, diagnostic testing and treatment for patients identified as high risk for stroke * lead by Dr Nida Riaz

    • consultations with internists
    • a prevention nurse
    • dietician
    • referral to diabetic education program
    • cholesterol class
    • referral to other services
    • diagnostic tests as needed



Fees: Most services covered by OHIP
Application: Physicians referral * referrals can be faxed to 613-345-8348 * patients are prioritized based on a needs assessment
Eligibility / Target Population: Individuals who have experienced a transient ischemic attack or stroke
