Service Description:

To provide flexible strengths-based and barrier free supportive housing for persons with a serious mental illness or dual diagnosis that is based on respect, dignity, hope and collaboration. Housing available in Peterborough, Kawartha Lakes and Northumberland.

To support to live as independently as possible
To support people to participate in the community, accessing community resources and assistance
To provide supports to minimize stress, provide illness management and prevent crisis and hospitalization
To provide education and support to people to enable them to develop and maintain skills and relationships.

Supports are flexible and based on client need and stability and may include: Case Management * Housing Supports * Housing Retention visits * Partnerships with community partners who provide the functions of primary support person in the unit.
