Service Description: Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible * referral to other community services

Support, education, and advocacy for families of people with Alzheimer's disease and other dementias
 
Bancroft Office - Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible * referral to other community services * provides information, education and support through home visits, phone calls, support groups and workshops
 
Brighton Area Support - One-on-one and group support, as well as counselling for caregivers and families * home office provides home visits and education sessions to help support people and their families with Alzheimer's disease or related dementias
 
Caregiver Support Groups - Peer support and discussion with educational video series, for family and friends who are caregivers 
 
Centre Hastings Area Support - Peer support groups, education and training assist family members, service providers and caregivers * sessions may include information such as advanced directives and older adult safety
 
Early Stage Support Groups - Peer support groups, education and training assist family members, service providers and caregivers * sessions may include information such as advanced directives and older adult safety
 
First Link Learning Series - Peer support groups, education and training assist family members, service providers and caregivers * sessions may include information such as advanced directives and senior safety
 
Memory Cafe - Social gatherings * loosely structured with a positive focus offering options of refreshments information gathering, creativity and more
 
Picton site - Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible * referral to other community services * counselling, resource library
 
Plate Mates - Social outing for people living with memory loss and their family and friends
 
Quinte West Area Support - Peer support groups, education and training assist family members, service providers and caregivers * sessions may include information such as advanced directives and older adult safety
 
TRENDS - Health Services Information Group - Informal group meets to discuss senior's health, healthy aging, managing memory and dementia * helps attendees navigate the healthcare system in Brighton * facilitated by health care professionals
