Service Description: One-on-one or group diabetes consultation * monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian on staff * grocery store tours * community presentations

Programming is offered in the communities of:
  • Napanee
  • Deseronto
  • Tamworth
  • Amherstview and other communities as needed

Fees: Free
Application: Call * no referral required
Eligibility / Target Population: Adults 18 years and older with pre-diabetes, type 1 diabetes and type 2 diabetes
