Service Description:

Family and community health * health professionals assess and treat non-life-threatening injuries or illnesses * referrals to other local health services and personal development groups
Community-led health teams develop programs to improve the social supports and conditions that affect long-term health * not a walk-in clinic
- maternal newborn services
- breastfeeding support
- sexual health, family planning and birth control
- sexually transmitted diseases
- pregnancy tests
- postpartum depression

Diabetes Education Program
- One-on-one or group diabetes consultation * monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices * registered nurse and dietitian on staff * grocery store tours * community presentations

Diabetic and Advanced Foot Care
- Diabetic Advanced foot care provided by a Registered Nurse in a health care setting *foot care includes care of corns, calluses, fungal nails, and filing and trimming of nails

Good Food Box
- Low cost fresh fruits and vegetables
|
Includes health care, counselling, health promotion, and illness prevention programs (not a walk-in clinic) * health team includes doctors, nurses, nurse practitioners, dietitian, diabetes educator, social worker, physiotherapist and Aboriginal nurse practitioner
Counselling Services - One-on-one, couple, and family counselling * open to people residing in the catchment area
Living Well Education and Exercise Program - teaching of ongoing self-care for people living with chronic diseases * for registered clients who are referred by their health care provider
What is Aboriginal Health? Program - a four week discussion series that explores how traditional First Nations' approach to health might be applied today
