Service Description: Transitional supports when being discharged from hospitals in Northumberland County

Service may include:
  • driving and/ or accompanying a client home
  • picking up medications and/or groceries
  • preparing a small meal
  • personal care and/ or homemaking services
  • referrals to other community support services if necessary

Fees: Free
Application: A member of the patient's health care team at Northumberland Hills Hospital or Campbellford Memorial Hospital completes a referral form and faxes it to 905-372-3898
Eligibility / Target Population: Seniors or adults with disabilities who have little to no supports in place on day of discharge from a Northumberland County hospital
