Service Description: Nutrition, transportation, in-home and wellness services to support the health and well-being of people in Northumberland County
 
Friendly Visiting - Volunteer provided on a one to one basis to visit on a regular basis * visit activities may be playing cards, walks, crafts, conversation * volunteers are screened * can provide service for persons with dementia
 
Home At Last - Hospital Transition Program - Transitional supports when being discharged from hospitals in Northumberland County
 
Home Help and Maintenance Program - Assistance with light cleaning * shopping * meal preparation
 
Hospice Palliative Care Program - Visiting Hospice Service - Trained volunteers provide emotional, psychological, spiritual and social supports
 
Meals On Wheels - Nutritious hot or frozen meals are delivered during the noon hour by volunteers to the person's home * special diets can be accommodated
 
Personal Distress Alarms - Medical alert system * allows older adults and people with disabilities to call for assistance using a help button * 24-hour emergency response
 
Specialized Transportation - Accessible and Rural Services - Transportation to medical appointments, shopping and social activities and programs
 
Support and Care for Caregivers - Assistance with the on-going responsibilities of caregivers who are caring for a loved one, friend or neighbor * peer support and discussion for caregivers and family and friends to share coping strategies and information on resources
 
Telephone Reassurance Calls - Regular telephone contact provided in order to provide reassurance and arrange for assistance if necessary * normally short five minute calls at regular, prearranged times or when family and friends are away
 
Wellness Programs - Social and recreational activities * program offerings vary
