Satellite location located within Community Home Support Lanark County
Prevention, education, and self-management support for adults living with diabetes or prediabetes * individual and group settings * education sessions are also available to community organizations and health care providers on request * service is provided at four locations as well as outreach locations throughout Lanark, Leeds and Grenville * provided by Registered Dietitians and Nurse Educators
