Service Description:

Medical care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, nurses, nurse practitioners, dietitian, and social worker

- nutrition education and counselling
- addictions counselling and treatment
- diabetes management
- foot care and wound care
- preventative care * colorectal cancer screening, immunizations, flu shots, paps and mammograms
- heart and stroke hypertension management program
- chronic obstructive pulmonary (lung) disease (COPD) management program, spirometry, smoking cessation
- phlebotomy (specimen collection / blood drawing) services on-site
- acute episodic care is provided to summer residents and visitors on triage basis
- prenatal instruction
- sexual health including pregnancy tests, family planning and sexually transmitted disease
- breastfeeding instruction
- palliative care home visits
- geriatric outreach home visits retirement home
