Service Description: Community-based support and rehabilitation services to adults living with the effects of an acquired brain injury or a developmental disability or a dual diagnosis (both a developmental disability and mental health or behaviour challenge) * helps people remain independent for as long as possible * referral to other community services

Pathways clients have access to counselling and psychiatric support

Fees: Fees depending on program
Application: Call * no referral required
Eligibility / Target Population: Adults 16 and older for Acquired Brain Injury (ABI) * adults 18 and older with developmental disability
