Service Description: Office for services in Nappanee and rural Frontenac and Lennox and Addington
 
Addiction Services - Outpatient counseling and treatment which may include individual, family, or group therapy for people affected by alcohol and/or drug use disorder * topics may include: 
  • techniques for abstention or controlled substance use
  • relapse prevention
  • harm reduction
  • communication skills
  • stress and anger management
  • lifestyle and relationship issues
 
Case Management - Case Managers work with individuals who have mental health or addiction issues and are living independently in the community.  Services are designed to assist clients in their home, work, school and social environments. Case Managers provide goal-oriented and individualized supports focusing on improved self-sufficiency, through assessment, planning, referrals, advocacy and the development of skills and resources that meet the individual’s needs. Meetings are confidential and can take place in the home, in a community setting (e.g. coffee shop), or at our offices. 
 
Counselling and Treatment Services - Evidence-informed psychotherapy for people with serious mental health issues * staff are guided by evidence-informed approaches including Cognitive Behavioural Therapy (CBT) * Dialectical Behavioural Therapy (DBT) as well as other approaches
 
Crisis Services - 24/7 comprehensive crisis response service * in-house/mobile crisis assessments and risk assessments, short term support and stabilization
 
Housing Services - 12 independent living bachelor apartments co-located with a main office site * five residents can be supported by Providence Care via the Community High Intensity Treatment Team ( CHITT ) * all clients participate in a comprehensive assessment process to determine skill level and suitability for apartment settings
 
Justice Services - Criminal justice program
  • assessments, treatment planning and follow-up to individuals that the crown approves for diversion
  • mental health consultation is provided to individuals referred via the courts who do not meet the diversion mandate
  • discharge planning services are offered to mentally disordered offenders
  • short and long-term case management
  • anger management groups are provided on an ongoing basis 
 
Problem Gambling Services - Public awareness presentations related to problem gambling * outpatient assessment, counseling and case management services to individuals experiencing problems related to gambling or gaming
