12 independent living bachelor apartments co-located with a main office site * five residents can be supported by Providence Care via the Community High Intensity Treatment Team ( CHITT ) * all clients participate in a comprehensive assessment process to determine skill level and suitability for apartment settings

Self refer or referral by a third party * referrals for psychiatric consultation must be from a family physician * clients must participate in a standardized Central Intake assessment to become clients The waiting period depends upon the service

Adults 16 years of age and older who have acute or chronic symptoms of a serious mental illness
